We have been pointing out many times that valve stem extensions can be an issue and if used they need to be in inspected for abrasion wear and the grommet has to be stiff enough to support the valve stem against deflection that is going to cause fatigue failure of the stem where it comes out of the wheel.
But look closely at the photo Tom posted. The hole to the right of the valve stem extension in the wheel shows where on that wheel there was previously an extension and the grommet was not in place and the extension wore a small groove in the opening in the wheel.
When we were parking coaches at the last Carlisle event about 10 or 15 of the coaches had missing or out of place grommets and those coaches were a tire failure about to happen.
