In answer to your question Jon, I’d say upwards of 90% of all incidents are self inflicted. Of course no one will admit their stupidity was the source of their incident.

Will, do you think the truck driver made a bad decision, and then followed it up with a second bad decision. Entering the limited length road was his first screw up, but then the moron kept going instead of stopping, realizing he was screwed and calling for help so he could back out or get a wrecker to help him get out of the situation.
Seeing the video it makes me wonder how with his 70 foot length he did not understand the much lower limit was far beyond his ability.
